{"id":257383,"date":"2024-06-05T13:21:20","date_gmt":"2024-06-05T11:21:20","guid":{"rendered":"https:\/\/invimed.pl\/?page_id=257383"},"modified":"2025-09-11T10:42:08","modified_gmt":"2025-09-11T10:42:08","slug":"digitalisation-of-medical-records","status":"publish","type":"page","link":"https:\/\/invimed.pl\/en\/digitalizacja-dokumentacji-medycznej","title":{"rendered":"DIGITALISATION OF MEDICAL RECORDS"},"content":{"rendered":"<section class=\"check-list\">\n\t    <header class=\"check-list__header\">\n        <div class=\"acf-innerblocks-container\">\n\n<h4 class=\"wp-block-heading\">At Invimed clinics, we digitise the paper medical records produced in connection with our treatment activities.<\/h4>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><\/p>\n<\/blockquote>\n\n\n\n<p>Pursuant to Article 13b of the Act of 28 April 2011 on the health care information system, a service provider may change the form of medical records kept and stored in paper form to an electronic form, hereinafter referred to as 'digitisation of medical records'.<\/p>\n\n\n\n<p>Digitisation consists of:<\/p>\n\n\n\n<section class=\"check-list\">\n\t    <header class=\"check-list__header\">\n        <div class=\"acf-innerblocks-container\">\n\n<p><span style=\"text-decoration: underline;\">The legal bases for processing your data are:<\/span><\/p>\n\n<\/div>\n    <\/header>\n    <ul class=\"mb-0 p-0 check-list__inner\">\n            <li class=\"d-flex check-list__item\">\n            <div class=\"svg-container\">\n                <svg width=\"16\" height=\"16\">\n                    <use xlink:href=\"#icon-check-2\" \/>\n                <\/svg>\n            <\/div>\n            <div class=\"check-list__item__rte\">\n                <p>making a digital reproduction (scan) of the medical records,<\/p>\n            <\/div>\n        <\/li>\n            <li class=\"d-flex check-list__item\">\n            <div class=\"svg-container\">\n                <svg width=\"16\" height=\"16\">\n                    <use xlink:href=\"#icon-check-2\" \/>\n                <\/svg>\n            <\/div>\n            <div class=\"check-list__item__rte\">\n                <p>bearing an appropriate form of signature<\/p>\n            <\/div>\n        <\/li>\n            <li class=\"d-flex check-list__item\">\n            <div class=\"svg-container\">\n                <svg width=\"16\" height=\"16\">\n                    <use xlink:href=\"#icon-check-2\" \/>\n                <\/svg>\n            <\/div>\n            <div class=\"check-list__item__rte\">\n                <p>and storage of such records in electronic form.<\/p>\n            <\/div>\n        <\/li>\n        <\/ul>\n<\/section>\n\n\n\n<p>The patient has the right to collect the paper medical records, once they have been digitised.
