Those who qualify for the Programme, gain a full range of benefits from examinations to reimbursement of drugs, the procedure or embryo storage.  An application for in vitro funding is available at any facility <\/strong><a href=\"https:\/\/invimed.pl\/en\/clinics\/\"><strong>Invimed clinics<\/strong>.<\/a><\/p>\n\n\n\n<p>As of 1 June 2024, the <a href=\"https:\/\/invimed.pl\/en\/in-vitro\/refund\/\">In vitro reimbursement programme<\/a>which allows all couples struggling with infertility, after meeting the eligibility criteria, to benefit from this modern treatment method free of charge.<\/p>\n\n\n\n<p>The IVF application is a document required in the process of qualifying for the government's IVF Support Programme. It contains all the necessary information concerning the couple who would like to benefit from free IVF treatment.<\/p>\n\n\n\n<p>The programme is aimed at couples, both married and unmarried, who meet certain age and medical criteria:<\/p>\n\n\n\n<section class=\"check-list\">\n\t    <header class=\"check-list__header\">\n        <div class=\"acf-innerblocks-container\"><\/div>\n    <\/header>\n    <ul class=\"mb-0 p-0 check-list__inner\">\n            <li class=\"d-flex check-list__item\">\n            <div class=\"svg-container\">\n                <svg width=\"16\" height=\"16\">\n                    <use xlink:href=\"#icon-check-2\" \/>\n                <\/svg>\n            <\/div>\n            <div class=\"check-list__item__rte\">\n                <p>age 42 for a woman and 55 for a man (IVF from own reproductive cells, IVF with sperm donation);<\/p>\n            <\/div>\n        <\/li>\n            <li class=\"d-flex check-list__item\">\n            <div class=\"svg-container\">\n                <svg width=\"16\" height=\"16\">\n                    <use xlink:href=\"#icon-check-2\" \/>\n                <\/svg>\n            <\/div>\n            <div class=\"check-list__item__rte\">\n                <p>45 years for a woman and 55 years for a man (if using IVF procedures with donor eggs or embryo adoption).<\/p>\n            <\/div>\n        <\/li>\n        <\/ul>\n<\/section>\n\n\n\n<p>The government programme provides full funding for IVF for up to six individual assisted reproductive procedures, including:<\/p>\n\n\n\n<section class=\"check-list\">\n\t    <header class=\"check-list__header\">\n        <div class=\"acf-innerblocks-container\"><\/div>\n    <\/header>\n    <ul class=\"mb-0 p-0 check-list__inner\">\n            <li class=\"d-flex check-list__item\">\n            <div class=\"svg-container\">\n                <svg width=\"16\" height=\"16\">\n                    <use xlink:href=\"#icon-check-2\" \/>\n                <\/svg>\n            <\/div>\n            <div class=\"check-list__item__rte\">\n                <p>Up to 4 cycles of fertilisation with own reproductive cells or sperm donation;<\/p>\n            <\/div>\n        <\/li>\n            <li class=\"d-flex check-list__item\">\n            <div class=\"svg-container\">\n                <svg width=\"16\" height=\"16\">\n                    <use xlink:href=\"#icon-check-2\" \/>\n                <\/svg>\n            <\/div>\n            <div class=\"check-list__item__rte\">\n                <p>Up to 2 fertilisation cycles with oocytes from donors - with the possibility of fertilising 6 reproductive cells in one cycle;<\/p>\n            <\/div>\n        <\/li>\n            <li class=\"d-flex check-list__item\">\n            <div class=\"svg-container\">\n                <svg width=\"16\" height=\"16\">\n                    <use xlink:href=\"#icon-check-2\" \/>\n                <\/svg>\n            <\/div>\n            <div class=\"check-list__item__rte\">\n                <p>up to 6 cycles with embryo donation.<\/p>\n            <\/div>\n        <\/li>\n        <\/ul>\n<\/section>\n\n\n\n<p>The programme finances the full range of in vitro fertilisation procedures, providing patients with comprehensive care at every stage of treatment - from medical qualification, through mandatory diagnostic tests, to the various phases of the IVF procedure, such as ovarian stimulation, puncture, oocyte fertilisation and embryo transfer and storage of frozen oocytes and embryos.<\/p>\n\n\n\n<p>To benefit from the government's IVF Support Programme, the couple must have a documented diagnosis of infertility. The requirement is to provide full medical documentation that confirms infertility or lack of effective treatment for at least 12 months prior to application for the Programme. This information is crucial and must be included in the application for <a href=\"https:\/\/invimed.pl\/en\/in-vitro\/funding\/\">subsidising the in vitro procedure.<\/a><\/p>\n\n\n\n<p>Couples interested in participating in <a href=\"https:\/\/invimed.pl\/en\/dofinansowanie-in-vitro\/\">In vitro support programme<\/a> should visit one of the clinics of Invimed, where they will obtain an application form for the financing of the in vitro procedure. This application should be filled out precisely, taking into account all the required data, including documentation confirming the infertility diagnosis and information about previous treatment attempts. In the event of difficulties in filling in the application or any procedural doubts, patients can seek the assistance of consultants available at the <a href=\"https:\/\/invimed.pl\/en\/clinics\/\">establishments<\/a>. These professionals are prepared to provide expert assistance, ensuring that documents are completed correctly and completely in accordance with the requirements of the Programme.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"jakie-informacje-zawiera-wniosek-o-dofinasowanie-in-vitro\">What information does the IVF application contain?<\/h2>\n\n\n\n<p>The application for government support for IVF contains a number of key pieces of information that are necessary to assess a couple's eligibility for the Programme and to carry out the procedure correctly. Here are the main elements that can be found in the application:<\/p>\n\n\n\n<p><strong>Personal data:<\/strong> Name, surname, date of birth, PESEL (if applicable in the country), residential address, telephone and e-mail contact.<\/p>\n\n\n\n<p><strong>Medical history:<\/strong> This section should include all information on chronic diseases, previous gynaecological treatments, past infections, diagnostic tests and details of previous attempts at infertility treatment.<\/p>\n\n\n\n<p><strong>Partner (if applicable):<\/strong> Information on the partner who will be involved in the procedure. Personal and medical data, including semen testing, may be necessary.<\/p>\n\n\n\n<p><strong>Research findings: <\/strong>It is useful to provide previous diagnostic test results prior to IVF, such as hormone tests, ultrasound, semen tests, genetic tests.<\/p>\n\n\n\n<p><strong>Declaration on early IVF trials<\/strong> - in cases where the couple has previously benefited from similar programmes, information on the results of previous procedures may be required.<\/p>\n\n\n\n<p><strong>Statements<\/strong> - The couple may be required to make declarations concerning, for example, the absence of health contraindications to undergo the IVF procedure.<\/p>\n\n\n\n<p>The IVF reimbursement application is an important document that enables proper medical and administrative qualification and ensures compliance with the formal requirements of the programme.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"gdzie-znalezc-wniosek-o-refundacje-in-vitro\">Where to find an application for in vitro reimbursement?<\/h2>\n\n\n\n<p>Applications for in vitro reimbursement are available from the <a href=\"https:\/\/invimed.pl\/en\/clinics\/\">clinics<\/a>which were qualified for the Programme.
