Take advantage of co-financing in vitro in the Invimed clinic in Warsaw, which is the realisation of the Health Policy Programme of infertility treatment by means of in vitro fertilisation for the inhabitants of the Mazovian Voivodeship. The health policy programme is co-financed from the budget of the Mazovian Voivodeship.<\/strong><\/p>\n\n\n\n

Eligibility criteria for participation in the programme for the use of in vitro fertilisation:<\/h2>\n\n\n\n
    \n
  1. Applicants to the Programme (male and female) must be at least 20 years old on the date of application;<\/li>\n\n\n\n
  2. persons applying for the Programme (a woman and a man) must live on the territory of the Mazowieckie Voivodeship, i.e. they must be natural persons residing on the territory of the Mazowieckie Voivodeship within the meaning of the Civil Code for the period of 12 months at the time of applying for the Programme. The place of residence is the place where the person stays with the intention of permanent residence. The fact of being registered in the territory of another voivodeship does not constitute a contraindication to participate in the Programme, as the declaration of residence on the territory of the Mazowieckie voivodeship is decisive;<\/li>\n\n\n\n
  3. applicants to the Programme (female and male) must submit to the implementer a completed informed consent form to participate in the Programme and sign a declaration that they are not currently participants in another Programme of a similar nature funded by public funds or have not participated in a similar Programme funded by public funds in the last three years;<\/li>\n\n\n\n
  4. in persons applying to the Programme (woman and man) the absolute cause of infertility or the ineffective treatment of infertility in accordance with the recommendations and standards of medical practice during the period must be established and confirmed by medical documentation 12 months<\/strong> preceding the application for the Programme, with an indication for in vitro fertilisation.<\/li>\n<\/ol>\n\n\n\n

    Eligibility for the programme is decided by the doctor, after reviewing the couple's medical situation.<\/h3>\n\n\n\n

      In women with impaired fallopian tube function with preserved patency or after microsurgery and 2 years without pregnancy. A condition for recommending waiting is that there are no other factors that may affect the chance of pregnancy (abnormal sperm, woman's age > 35 years, duration of infertility <3 years, endometriosis, ovulation disorders).<\/p>\n <\/div>\n <\/li>\n <\/ul>\n<\/section>\n\n\n\n

            Endocrine disruption up to 6 cycles stimulated without effect.<\/h3>\n\n\n\n

            Unsuccessful attempts at intrauterine insemination for up to 3 cycles.<\/h3>\n\n\n\n

            All of the above criteria in points 1-3 and at least one criterion from point - Treatment Eligibility Criteria. Failure to meet at least one criterion from points 1-3 or any criterion from point - Treatment Eligibility Criteria constitutes a contraindication to participation in the Programme.<\/p>\n\n\n\n
