The Invimed Clinic in Wroc\u0142aw has won another competition for the 'Programme for infertility treatment using in vitro fertilisation for the inhabitants of Wroc\u0142aw'.<\/strong><\/p>\n\n\n\n

THE PROGRAMME IS FINANCED BY THE CITY OF WROC\u0141AW.<\/mark><\/sup><\/p>\n\n\n\n

<\/p>\n\n\n\n

Thanks to further funding from the Municipality of Wroc\u0142aw for 2024, couples from Wroc\u0142aw will be able to benefit from reimbursement of the treatment. The programme will subsidise the cost of one cycle of in vitro fertilisation* for married or partnered couples who qualify for the Programme. <\/p>\n\n\n\n

Maximum the amount of funding is PLN 10 000<\/strong>, provided that at least one entire procedure under the programme has been carried out.<\/p>\n\n\n\n

Eligibility criteria for the programme<\/h3>\n\n\n\n

To qualify for the Programme, the following criteria must be met:<\/p>\n\n<\/div>\n <\/header>\n

    established cause of infertility or ineffective treatment of infertility in the 12 months preceding application to the Programme,<\/p>\n <\/div>\n <\/li>\n

    \n

    on the day of enrolment in the Programme, the woman is under 40 years of age, but it is possible to carry out the procedure in patients up to 42 years of age (according to the year of birth) on condition of promising ovarian reserve, confirmed by AMH examination above 0.7 ng\/mL) or in the case of couples carrying out the insemination procedure within the framework of non-partner donation (female germ cell donation) or the embryo adoption procedure,<\/p>\n <\/div>\n <\/li>\n

    the couple is married or in a civil partnership,<\/p>\n <\/div>\n <\/li>\n

    on the day of qualification for the Programme, both are residents of Wroc\u0142aw and settle their income tax at Wroc\u0142aw Tax Offices (excluding the 1st Tax Office at ul. Klimasa 34, 50-515 Wroc\u0142aw).<\/p>\n <\/div>\n <\/li>\n <\/ul>\n<\/section>\n\n\n\n

    Attention <\/mark>

    The decision on the eligibility of a couple for infertility treatment with the in vitro fertilisation method is taken by the Programme Provider, taking into account the statutory conditions for undertaking in vitro fertilisation treatment and other criteria included in the Programme.<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n

    Programme registration<\/h3>\n\n\n\n

     In order to register for the Programme, each person from a married or partnered couple must submit the following documents:<\/p>\n\n\n\n

      \n
    1. Consent to participate in the Programme.<\/li>\n\n\n\n
    2. A statement confirming participation in the Programme for the first time.<\/li>\n\n\n\n
    3. Statement of residence in Wroc\u0142aw.<\/li>\n\n\n\n
    4. A photocopy of the first page of the tax return for 2023. - by each of the beneficiaries making up the couple who have settled their accounts with the Tax Office by the date of submission of the documents ( original return to be inspected). Those who have not settled, submit a photocopy of the first page of the PIT-11.<\/li>\n\n\n\n
    5. Declarations of consent to the processing of personal data for the purposes of the Programme,<\/li>\n\n\n\n
    6. Declarations of being married or in a civil partnership (in accordance with the definition of civil partnership donation set out in art.2 para. 1 pt. 8 of the Act of 25 June 2015 on Infertility Treatment).<\/li>\n\n\n\n
    7. Other, including medical, as required by the Programme Implementer.<\/li>\n<\/ol>\n\n\n\n

      Medical qualification is carried out by the Programme provider - in accordance with the guidelines of the Polish Society of Reproduction and Embryology, the Polish Gynaecological Society and the provisions of the Act of 25 June 2015 on Infertility Treatment (Journal of Laws 2017, 865).<\/strong><\/p>\n\n\n\n
