DIGITALISATION OF MEDICAL RECORDS
At Invimed clinics, we digitise the paper medical records produced in connection with our treatment activities.
Pursuant to Article 13b of the Act of 28 April 2011 on the health care information system, a service provider may change the form of medical records kept and stored in paper form to an electronic form, hereinafter referred to as 'digitisation of medical records'.

making a digital reproduction (scan) of the medical records,

bearing an appropriate form of signature

and storage of such records in electronic form.
The patient has the right to collect the paper medical records, once they have been digitised. Documents not collected by the patient will be kept for a period of 1 year and after this time, without further notice to the patient, will be destroyed.
The medical information presented should be considered as general guidelines and does not replace the individual judgement of the doctor regarding the medical management of each patient. The doctor, after a thorough examination of the patient's condition, determines the extent and frequency of diagnostic tests and/or therapeutic procedures, taking into account specific medical indications. All medical decisions are made in full consultation with the patient.
