One of the methods available for the diagnosis of deep infiltrating endometriosis is an ultrasound examination performed according to the extended IDEA protocol by a doctor trained in the diagnosis of this condition. The study is aimed at patients with suspected deep infiltrating endometriosis who present with symptoms such as:

Diagnosis of endometriosis - what is sonovaginography

Sonovaginography is an extended specialised ultrasound examination of endometriosis. It can be performed with the use of vaginal gel contrast. The examination is performed when superficial and deeply infiltrating endometriosis foci are suspected. Sonovaginography is performed with a transvaginal probe and partially with a transabdominal probe. Thanks to the use of a special gel that acts as a contrast agent, a more accurate and precise assessment of the vaginal and rectal walls is possible. In addition, the protocol for performing this examination also includes the assessment of the sacro-uterine ligaments, bladder wall, ureters and bowel, which is not routinely assessed during traditional screening gynaecological ultrasound.

What is sonovaginography?

Ultrasound examination of endometriosis with gel contrast can detect foci of superficial and deeply infiltrating endometriosis.

Sonovaginography - what does the examination look like?

  • Sonovaginography is performed using transvaginal and transabdominal probes.

  • The examination is preceded by a medical history, i.e. the collection of information about ailments.

  • Before the examination, the doctor uses a special catheter to insert a gel into the vagina, which acts as a contrast agent. This allows him to identify endometriosis on ultrasound in areas that are less visible than during a routine examination.

  • In the first stage, the kidneys are examined using a trans-abdominal probe to exclude possible urinary stasis, which can be caused by ureteral stricture in the course of endometriosis.

  • As with the screening ultrasound, the doctor assesses the reproductive organs, i.e. the cervix, the endometrium and the ovaries. In addition, the mobility of these organs in relation to each other and the surroundings is assessed.

  • This can be followed by a thorough assessment of the area around the vaginal walls, bladder, ureters, sacro-uterine ligaments, rectovaginal septum and rectum and terminal ileum for foci of deeply infiltrating endometriosis.

An ultrasound of endometriosis with gel contrast takes longer than a standard gynaecological examination. Due to the fact that deeper structures are assessed, discomfort may be experienced.

Who should perform sonovaginography and when?

Sonovaginography is recommended for patients with suspected deep infiltrating endometriosis. The examination is used to assess the progression of the disease. This is the most severe and advanced form of this disease.

Preparing for endometriosis ultrasound - what do you need to know?

The examination can be performed at any time, but is best performed in phase I of the cycle, just after menstruation. Unlike the classic gynaecological screening ultrasound, the bladder should be slightly full, at least in the initial stage of the examination.
