PICSI® (Physiological Intracytoplasmic Sperm Injection)

PICSI® insemination is a technique that uses the physiological properties of mature and properly structured sperm to bind to hyaluronic acid, thanks to the presence of receptors for hyaluronate on the surface of the sperm head.

How is sperm selected in the PICSI method?

In the PICSI® method, spermatozoa are selected on the basis of morphological criteria and a positive and specific sperm-hyaluronate binding reaction.

Normal, motile spermatozoa react with the hyaluronate matrix of the oocyte's ovipositor thalamus through specific receptors located on their head. The binding of the sperm to hyaluronate suggests that the selected sperm has undergone the entire process of spermatogenesis and spermiogenesis correctly, and is biologically mature, fully functional and competent to fertilise the egg.

What study helps determine whether PICSI will make sense?

In order to select a fertilisation method, appropriate diagnostics are necessary. The basic eligibility test for insemination by the PICSI method is HBA test.

The HBA test determines the percentage of biologically mature sperm. This test is crucial to determine whether sperm are competent to fertilise a cell during traditional efforts, insemination, or standard IVF.

An abnormal HBA test result with a significantly reduced percentage of mature sperm suggests the choice of fertilisation by ICSI or PICSI®.  

Indications for in vitro fertilisation using the PICSI® method:

  • recurrent miscarriages,

  • unsuccessful attempts at pregnancy with standard IVF or ICSI (few developing and low-quality embryos),

  • repeated failure of transfers related to difficulty in implanting the embryo despite medication,

  • abnormal HBA test result, SCD,

  • severe asthenozoospermia in semen examination,

  • ICSI procedure using frozen low-performance sperm or sperm suspension obtained by TESA, PESA, MESA.

When HbIMSI?

In case of teratozoospermia or an abnormal result MSOME, our clinic offers the HbIMSI insemination method, a combination of two methods - PICSI® and IMSI. 

HbIMSI allows the selection of motile spermatozoa on the basis of their ability to bind to hyaluronic acid and morphology under high magnification as in the IMSI method.

With HbIMSI, mature sperm and sperm with the best morphology are selected for fertilisation, and couples can achieve a higher fertilisation rate and better quality embryos.
